Identity Proof [PAN Card, Passport, Driving License, Voter Card, Aadhar Card etc.]
Address Proof [Ration Card, Passport, Driving License, Aadhar Card etc.]
Certificate showing Date of Birth [10th Certificate/Birth Certificate]
Two Recent passport size photographs.
Photocopies of Aadhar Card/PAN Card and Bank Passbook [At the time of joining for only selected candidates]
Class 10th& 12th Mark Sheets or Certificates.
MBBS, MD/MS/DNB and Experience certificate whichever is applicable as per NMC guidelines.
Registration with National Medical Commission / State Medical Council [For Medical Candidates]
Subsequent to the selection, the candidate has to register with Tamilnadu State Medical Council not later than 30 days from the date of joining if not registered at the time of joining.
Internship Completion Certificate.
Reservation category Certificate [OBC*/SC/ST/PH/EWS] in the format prescribed by Govt. of India (* Candidate should belong to non-creamy layer of central lists of OBC).
NOC (If applicable)
Demand draft for Interview Fee of Rs.500/- in favour of ESI Fund A.C No.1, payable at Chennai. Fee once remitted shall not be refunded under any circumstances
